Marriage Request Form

  • Although the title of this form is “Request for Marriage”, this is an all-inclusive information gathering form. We use this online tool for anything related to the sacrament of marriage at our parish. If you are looking for annulment, convalidation, marriage prep (because you are having your wedding somewhere else), or a wedding here… whatever the circumstances are — this is the form you need to fill out first.

    Once this form is completed you will be able to access our online ceremony scheduler to submit your preferred date and time.

  • ABOUT THE BRIDE

  • Please describe your faith life and how often you attend Mass, go to Confession, private prayer. Please be honest. If you are not Catholic, please tell me about your faith life and what religion or denomination you practice. How often?

  • NOTE: If NO, you will need a letter from your Pastor granting you permission to marry at our parish and stating that they have been informed as to where the marriage preparation will be.

  • For any previous marriages, please use the box below to provide detailed information concerning those relationships. Some items you should include are: How you were married? Was it civilly, in another church? How long were you married to this person? Did the marriage end in divorce, annulment or death? If you were married multiple times, this is where you can let us know specific information that was not captured by the questions in the form above. Remember: we will need copies of your divorce decree or annulment.
