Registration is by your child’s GRADE.

  • Registration is by your child’s grade level.  If a grade level does not exist on the child’s record or the grade level is wrong, the event will not permit you to continue.  If this happens to you, stop and UPDATE YOUR HOUSEHOLD first.
  • To prepare for online registration, take a moment to review/update ALL your contact information. We use the email addresses and phone numbers on your (and your child’s) profile to communicate with you about your child’s program.

Elementary Faith Formation (K-6th)

Elementary Faith Formation is for children in Kindergarten through 6th grade. 

Our Elementary Faith Formation program will be structured as remote-study with online components, workbooks, and parent/guardian-led instruction that features fully individualized learning on a flexible schedule. This will be a great year to remind ourselves, as the catechism does, that parents are indeed, the primary catechists of their children.


The Christ in Us curriculum your student will be using combines solid catechetical vision with innovation and support for families. This K–6 program offers the choice of a comprehensive distance learning experience or a blended learning approach that integrates print and digital into a seamless religious education experience. NOTE: A physical book as well as online access will be included with the registration fee.

Junior High School Formation (7th-8th)

Junior High Faith Formation is for teens in 7th and 8th grades.


This may be a challenging year, but we are here for you! We’ve modified the wonderful textbook, Living as a Disciple of Christ, from Sophia Institute Press, to focus on the 18 core sessions. By following the suggested teaching schedule, you and your child can complete the program before Spring Break.


We will also assist parents by creating videos to help and support you as the primary catechist for your child, as well as videos for your 7th or 8th grader. We are excited to launch this program which will encourage, equip, and empower you as a parent to walk alongside your junior high teen throughout this year!

High School Formation (9th-12th)

High School Faith Formation is for teens in 9th through 12th Grades.


Fall Semester

All Youth Ministry teens will begin the year in the same way with 6 recorded sessions. These six sessions will be presented by Mike Gormley via recorded videos and will address belief in God and the truth of our Catholic faith. They will be asked to complete corresponding questions by December 1.


Spring Semester

▸ 9th-11th Grade, not confirmed yet, will be a digital format with assessment/comprehension questions to be submitted online.
▸ 10th-12th Grade, already confirmed, will be placed in gender-specific small groups of 8-10 members will meet via online video classes beginning in January. These groups will be facilitated by Core members. Small group meetings will be enhanced with the Discipleship Group Workbook, which is included in the program fee.


Those who are seeking Year 1 of Confirmation Preparation, should register for High School Formation. This will fulfill your Year 1 requirement, allowing your teen to be eligible for Year 2 of Confirmation.

First Sacraments Preparation (2nd-8th)

First Reconciliation / First Communion is for children in 2nd through 8th grades who have met the pre-requisite criteria.



Baptized Catholic children in the 2th—8th grades who have completed one year of Faith Formation classes will prepare for First Reconciliation and First Communion at home with their parents. Parents will be given all the necessary resources and support.


Requirements for registration:
• Be in at least the second grade.
• Have completed the previous year’s weekly Faith Formation classes.
• Be registered for and attending the current year’s weekly Faith Formation classes.
• Be a Baptized Catholic. Copy of child’s Catholic Baptism Certificate must be turned in during registration.
• Be attending Mass regularly (weekly).


NOTE: If your child is in 2nd – 6th grade, you must first register using the Faith Formation for Kinder thru 6th form.  If your child is in 7th or 8th grade, you must first register using the Junior High Youth Ministry form. All registration submissions will be reviewed.  If your child meets the pre-requisite requirements, a link to event registration for the First Sacraments program will be sent to your family individually.

Teen Confirmation Preparation (10-12th)

Confirmation is for teens in 10-12th grades who have met the pre-requisite criteria.



We prepare high school teens over a two year period for the Sacrament of Confirmation. Teens are brought further in their journey of knowing the love of Jesus. They are also prepared to receive the Sacrament of Confirmation in order to live out the graces received through the Holy Spirit to participate in the life and mission of the Church.

4. I am going to volunteer with the faith formation programs, is there anything I need to do?

YES! As part of the Archdiocese’s commitment to ensure the safety of our children, the Archdiocesan Safe Environment Program requires that clergy, staff, and volunteers (ages 18 and above) who have regular contact with children and youth are required to complete the CMG Safe Haven online training program.


This training consists of 3 vignette style training videos that is approximately 90 minutes and will include a criminal background check that must be completed. These trainings are available in English and Spanish. The Diocese will no longer offer live trainings, all Safe Environment trainings will only be available online.



St. Anthony of Padua requires all adults volunteering with our parish be in compliance with the Safe Environment qualifications.

• If you do not have a VIRTUS account: Log into, and follow the instructions. Note: Your legal name (on your driver’s license/passport) is the name that one needs to use when registering. Not doing so may prohibit your information from being processed.



You will be required to take the training every 5 years. Please make sure your email address stays up to date at the CMGConnect website since this is how training notifications will be sent.
